Accepting a place in our school

Welcome to our school!

This page is for parents and guardians of children who have received an offer of a school place in Citywest ETNS. If you do not wish to accept the school place, you do not need to complete this form; simply reply to the email titled 'URGENT & IMPORTANT Offer of a school place' to let us know that.

If you wish to accept the school place, please complete the form below. This information will be uploaded to the Pupil Online Database, which is operated by the Department of Education (DES). The information will also be uploaded to the school information management system in use by our school.

You also need to submit a copy of your child's birth certificate to the school office. Both of these actions must take place by the time and date given in the email.
